Pickup and Walker Dismissal Forms

Please complete this form if you will regularly pick your student(s) up at dismissal. Note that you are not permitted to park and walk up to the building. Rather, you must remain in your vehicle. The vehicle pick-up line will be along the curb in front of the building. We will assign a number to your student(s) and provide corresponding cards for your student's backpack for you to hang from your vehicle's review mirror. Each family will be provided with 3 copies of the pick-up number to accommodate families with multiple vehicles.

If you would like your student to walk/bike unaccompanied to and/or from school daily, instead of riding the school bus or being picked up in a vehicle by a parent/guardian, this form must be completed. Paper copies will be available for families who are unable to complete this form electronically.
